Screening Verification

For my safety and security, I require references from two established providers, or a discreet identity and employment verification. Because of the high level of trust that my work entails security and discretion are essential, for us both. After we have met I will ask you to give me a unique nickname to remember you by, then I will destroy your contact information and remember you by that name. If you forget your nickname and cannot jog my memory, you will need to verify as if you are a new friend!

References

When providing references, please notify them in advance to remind them of your past meeting, and to assure them that my inquiry is authorized. I will need you to share with me their name, web address, and anything that might help them recall having met you. Ex. "She knows me as Mike from Chicago, I gave her a Norah Jones CD when I saw her last May."

No-Reference Screening

If you have no references you must provide me with the requested information below. It is easiest to copy and paste the text into your email to ensure that you submit a complete request. You are welcome to call and leave the information on my voice system. Please know that I do not answer calls, it is a voice mail system only. I will not return messages that are incomplete. For your safety I will not return your call unless you tell me when and how you would like that contact made.

• Comments, Suggestions, Requests… While not required, please feel free to share more about yourself as well! It is nice to know your age range, on-line handle, and where you found me. Please also let me know if you have any health concerns, allergies or sensitivities I should be aware of

Alternate Screening Options
